Here is the evidence for my 5 a Day reality check, below is the Data collection form and the reality check in the forms both with and without the formulas showing. The reality check posed a challenge with layout rather than with the actually making of it. Coming up with a design so that the reality check didn't simply look like a spreadsheet took a long time and a lot of drawing and re-drawing the whole design as is obvious from the evidence of change link.
